How to Apply for Housing?

Reach out to our team by phone or through our online apply form. A staff member answers your questions right away. We explain the program and next steps clearly.
Complete Referral Form
Submit a referral form with basic information. Referrals can come from you, a family member, or a case worker. We review each referral within a set timeframe.
Meet Case Manager
Once approved, you meet your dedicated case manager. This meeting covers your needs, goals, and housing options. Your case manager stays with you through the entire process.
Move In To New Home
You move into your new home once a unit becomes available. Your case manager helps with the transition. Support continues after move-in through wraparound services.
